Prescriptions
All prescriptions are done electronically (RxNT) and pharmacies should request refills electronically. Refill requests can be made by patient or pharmacy via e-mail or calling office or cell phone (be certain to leave message if you get voice mail).
For some controlled substances: including many pain medications and the following anticonvulsants clobazam (Onfi, Frisium, Urbanyl), clonazepam (Klonopin), diazeam (Valium), esogabine (Potiga), lacosamide (Vimpat), gabapentin Neurontin), lorazepam (Ativan), midazolam (Versed), pregabalin (Lyrica) most pharmacies are set up to accept Token Pin Authentication thus can accept all prescriptions electronically. A few compassionate programs (such as the one for Onfi) and UK and Canadian pharmacies require paper Rx's.
Patients who live in Florida and whose medication is covered by Medicaid at times may need a local physician to write the actual prescription in order to be reimbursed.
Requisitions for Tests/Test Results
If you need a requisition for a blood test, DXA bone density, renal ultra sound, liver ultra sound, echocardiogram, EKG, EEG, MRI etc this can be e-mailed to you as a PDF or faxed directly to the lab (we will need their fax number). Please request requisitions by calling the office at 203-987-3117 or e-mailing Mary Liz at Merrylizard@gmail.com. It is helpful for us if you would send a copy of your current insurance card, in case the test requires insurance approval.
Fasting is only necessary if cholesterol, triglycerides or fasting glucose is being checked. However, anticonvulsant levels should be drawn as close to 12 hours after the last dose as possible. For drugs taken once a day using an extended release preparation the level should be done 24 hours after the last dose. If the question being addressed is side effects caused by medication then at times it is preferable to have the blood test done when the side effects are being experienced. If levels are being checked because of a recent seizure blood should be drawn as close to the seizure as possible (this commonly requires going to local emergency department. Requisitions for blood tests are given at each appointment and it may be easiest to keep these in your wallet so if need be they can used at any time. If checking medication box demonstrates unequivocally that medication has been missed then level is not needed but please alert office.
DNA testing done on buccal swabs not blood. Patients should do their own swabbing to avoid contamination. If swabbing done by parent or caretaker then gloves should be worn.
If you wish a copy of the results ask us to write "copy to patient" on the requisition. Alternatively we can fax or e-mail you a copy when they arrive at the office.
Please try to have blood tests done at least 10 days before appointments.
