Please e-mail or fax intake form (see section under forms) and obtain: recent notes from physician responsible for prescribing anticonvulsants along with recent MRI's, EEG's, video-EEG, Neuro-Psych testing and blood tests.
Please have available photographs of actual bottles for all active medications and supplements (required for new medication reconciliation guidelines).
The intake form asks for a description of the seizures or other episodes in question. This information is crucial if an accurate paroxysmal episode/seizure/epilepsy diagnosis is going to be made. Videos of the events (see suggestions section) are the most helpful.
Allow at least a week for results of blood tests done prior to the visit to be available.
Dr. Resor does not participate with any insurance including Medicare (patients who qualify for Medicare need to sign a contract every two years declaring that they will not attempt to bill Medicare for Dr. Resor’s services). He does not charge for visits but rather all patients are charged a yearly fee of $1300.00 for access (e-mail/cell phone) and management of their seizure disorder/epilepsy. This is known as Concierge Medicine, (also known as retainer medicine), a relationship between a patient and a physician in which the patient pays a yearly fee or retainer. In exchange for the retainer, doctors provide an enhanced level of care, including a commitment to limit patient loads to ensure adequate time and availability for each patient. He is available almost all the time by phone and e-mail.
An initial consult costs $1,000.00 and covers the initial visit, review of all records, and the formulation of a plan with the patient, usually about 3 weeks of care. After that, should the patient wish to continue with Dr. Resor, it is $1300.00 a year.
We would also like a copy of your insurance information. While we do not participate with any insurance programs, we may need to get prior authorization for tests and medication, and having this information will expedite the process.
